Splenectomy in an uraemic patient with acquired factor X deficiency due to AL amyloidosis.

چکیده
The clinical signs of amyloidosis are dependent on the organ systems involved [I]. A common finding is renal involvement which manifests as nephrotic syndrome. Cardiac involvement may result in congestive cardiomyopathy with conduction defects and arrhythm ias. Amyloid deposits especially in the spleen may bind clotting factors , which can lead to coagulation abnormaliti es. Factor X deficiency is an unusual compl ication of the amyloid light-chain type (AL) of the disease [2]. We report a case of a patient in whom splenectom y was initially successful. The case illustrates the fluctuations of factor X levels before and after splenectomy.

Factor X deficiency: An uncommon presentation of AL amyloidosis
Factor X deficiency is the most common coagulation factor deficiency amongst patients with AL amyloidosis. It presumably occurs due to adsorption of factor X to amyloid fibrils. The deficiency of this factor, in conjunction with other hemostatic defects, can cause bleeding complications. A case of acquired factor X deficiency due to AL amyloidosis is reported, where abnormal coagulation paramet...

Amyloidosis is a heterogeneous group of diseases in which misfolding of extracellular proteins is the pathogenic factor. Light chain amyloidosis (AL) is the most common form of amyloidosis, and the causative proteins in AL are the immunoglobulin light chains produced by clonal plasma cells. Hemorrhagic events, ranging from mild subcutaneous hemorrhage to life-threatening bleeding, account for a...

Acquired defects other than iatrogenic are usually the result of liver dysfunction and in such cases more than one clotting factor is depressed. A case of selective factor X deficiency developing in middle life and associated with extensive amyloidosis has ...

متن کاملAcquired factor X deficiency in patients with amyloid light-chain amyloidosis: incidence, bleeding manifestations, and response to high-dose chemotherapy.
Acquired deficiency of factor X occurs in patients with systemic amyloid light-chain (AL) amyloidosis, presumably due to adsorption of factor X to amyloid fibrils. Of 368 consecutive patients with systemic AL amyloidosis evaluated at Boston Medical Center, 32 patients (8.7%) had factor X levels below 50% of normal. Eighteen of these patients (56%) had bleeding complications, which were more fre...
